Wasn't sure where or who to ask so I'm starting here. What if 2 families want to switch homes? Can this be done without a real estate agent or lawyer? What would have to be done if anything other than moving into the other home?

I wouldn't recommend doing it without an attorney. I know that it can be done, because I used to work at a title company. It's alot more complicated than just switching homes. Out here in Missouri, most title companies are owned by attorneys so things get done properly. I would check in your area and see what the requirements are.
