Do I just scan a copy of my birth certificate and a copy of my ID on one sheet of paper and then get it notarized?

No, you must take them with you to the notary so they can view them as proof of employment authorizanion, let the notary fill in the form, the make a copy of the legal docs and fax everything to HR, rmember you are under a deadline..... hope that helps.

crap... I didnt see your reply until after I went to the Notary, I didnt have the form, but she typed basically the same thing that was on the form on a copy of my documents. Do you think that will be sufficient?

i would say it depends on the HR staff that recieve it.... if there really is an HR dept there....

nevermind... they do want the right form. I am going to do it tomorrow since it is too late today!
