Are you going to connect your opt-in form to an autoresponder service provider? If so, they will tell you how to configure your form code. I can also help you with that, if you need my services!

Its free and the customer never gets directed to any advertising or anything like that. You can completely customize this form and I have even used it on sites I have created for others.
