what do you all use as an order form for the customer to fill out. The one on the website has old prices.

Hi Denise,
I use the one in the brochure alot. I have also gotten some general ones from my local office supply store.

Just like Natalie, I like the 3-part ones at Office Max or Office Depot - they work great because I can keep a copy for myself for the books, a copy for my follow up cust service file and a copy for their receipt.
