BONUS is available for shoppers who submit completed report
by assigned due date.
YOU MUST CHOOSE A TIME TO SHOP, EITHER BEFORE 5PM M-F, AFTER
5PM M-F OR ANYTIME ON THE WEEKEND
Based upon your selection of frames and lenses, there will
most likely be an out of pocket expense as a result of this shop. </span>
</span>*Shopper MUST have a
current (less than 1 year old) prescription for corrective lenses before
shopping. </span>If you do not have a current
prescription, you may use your own doctor or the doctor next to the Optical
shop to obtain one before completing this shop. </span>

to make a purchase of one or more pairs of prescription eyeglasses or prescription
sunglasses. </span>Contact lenses are not
allowed. </span>The evaluation is based on
purchase visit only. </span>
*As a condition of accepting this assignment you are
expected to keep the eyewear, not return the eyewear for a credit. </span>
*If you are using insurance, please verify that it is still
accepted by this Optical Shop. With the new year, many insurance companies have
changed allowed providers.
*You may purchase the glasses for another adult or child
ages 12-17 (you must be the parent, grandparent or legal guardian). </span>You must be present through the entire shop.
